Integrated Discharge Team 2019

A team involving representatives from the Trust, as well as partner organisations including Rotherham Council, won the Acute Service Redesign category at the HSJ Value Awards on 23 May.

The award is in recognition of their work to look at how partners across health and social care in Rotherham can work even closer together to provide effective discharge management, ensuring patients have the care and support in place to enable them to return home as soon as possible.

In total, four healthcare teams from The Rotherham NHS Foundation Trust had been shortlisted in the 2019 HSJ Value Awards across three categories.

Specialist colleagues from Nutrition and Dietetics and Speech and Language Therapy were highly commended in the Estates and Facilities Management award category for the way they have worked together with catering teams, food hostesses and others to ensure the Trust meets the high quality nutrition and hydration needs of patients.

The Clinical Imaging team was also highly commended in the Workforce Efficiencies category for redesigning their service to enable advanced Allied Health Professionals (AHPs) to report on plain film x-rays. This reduces the clinical risks of delayed diagnosis and improves the patient experience, as well as reducing the overall cost per report.

The nurse-led Ambulatory Care Team were also finalists in the Workforce Efficiencies category for their work in using acute medicine nurses and Advanced Nurse Practitioners (ANPs) to move a significant percentage of urgent care activity out of inpatient beds and reduce the amount of time patients spend in hospital.

Louise Barnett, Chief Executive at The Rotherham NHS Foundation Trust, said: “It is really fantastic news that our Integrated Discharge Team have won a national award for the service they provide to our patients and the people of Rotherham. The team works across health and social care with our partner organisations, particularly Rotherham Metropolitan Borough Council, to ensure patients are able to be discharged home in a timely way with the right level of support available to them. Well done to our fabulous team and to all our colleagues across Rotherham who work effectively with them to support our patients.

“I am also extremely proud of our teams who were highly commended in their categories and to our finalists. This is also an amazing achievement. They work extremely hard to ensure our patients receive the best possible care and are constantly looking at how they can improve patient experience. The teams were up against other strong contenders and so it is brilliant to receive national recognition for their efforts.”

The HSJ Value Awards seek to recognise and reward outstanding efficiency, improvements in patient care and reduction in variation. All submissions are reviewed against five criteria (ambition, outcomes, spread, value and involvement), with the highest scored making the shortlist. Shortlisted nominees then have to present their achievements to a panel of judges who determine the winners.
