Patients, carers and colleagues are being urged to nominate their health care heroes for recognition in The Rotherham NHS Foundation Trust’s 2015 PROUD Awards.  

The awards celebrate excellence in healthcare by recognising Rotherham’s exceptional NHS colleagues who pull out all the stops to deliver first class community and hospital services across our borough.

Following the tremendous success of last year’s awards, the Trust is again delighted to invite patients and members of the public to nominate the Trust’s healthcare professionals for a 2015 Public Recognition Award.

Louise Barnett, Trust Chief Executive, said: “Our Public Recognition Award provides a fantastic opportunity for members of the public to nominate individuals and/or teams who have made a real difference, delivering outstanding care to them or one of their loved ones.

“The introduction of this category was really well received last year with over 100 nominations from our patients and members of the public. Please take this opportunity to share with us your amazing examples of how our colleagues excel in their roles, living our values of responsible, compassion, respect, together, right first time and safe every day. I look forward to reading this year’s nominations.”

The Trust’s colleagues, eligible to be nominated for awards, deliver services at Rotherham Hospital, Rotherham Community Health Centre, BreathingSpace, at health centres across our town and includes those working in  people’s own homes and communities such as school nurses and district nurses.

In addition to the Public Recognition award, those who work at the Trust can nominate their colleagues in a number of additional categories including Team of the Year, Unsung Hero and Lifetime Achievement. Winners of each category are then eligible to receive the special Chief Executive or Chairman’s Awards.

Alongside the awards, in November, the Trust will run a week of celebration to mark the dedication and achievements of all those at the Trust. The week of events will include a ceremony in recognition of learning for all those who have completed qualifications over the last year, a ‘thank you’ scheme and long service celebrations.

To nominate a member of staff for the 2015 Public Recognition Award, simply print and complete this: Nomination Form or call 01709 426460 to have a paper form posted out to you.

Please complete the form with as much detail as possible, providing the name of the individual or team and specific examples of your experience. The closing date for nominations is 30th September 2015 and the official PROUD Awards ceremony will take place in November.
