The Rotherham NHS Foundation Trust Proud Awards 2018

Patients, carers and colleagues are being urged to nominate their health care heroes for recognition in The Rotherham NHS Foundation Trust’s 2018 Proud Awards. 

The awards celebrate excellence in healthcare by recognising Rotherham’s exceptional NHS colleagues who pull out all the stops to deliver first class community and hospital services across our borough.

To start our celebrations off,  we are pleased to officially open nominations for the Proud Awards 2018.

This year there is the opportunity to nominate within 16 categories:

Public recognition award
An outstanding individual, volunteer or team nominated by patients, visitors or members of the public

Team of the year (clinical)
An outstanding clinical team nominated by their colleagues

Team of the year (non-clinical)
An outstanding non clinical team nominated by their colleagues

Our top leader
An outstanding leader nominated by their colleagues

Unsung hero
An outstanding individual or team nominated by their colleagues

Outstanding volunteer award

An outstanding volunteer/s nominated by staff or a fellow volunteer

Learning and Development award
An outstanding individual/team nominated by their colleagues, teachers, mentors or supervisors

Lifetime achievement award
An outstanding individual nominated by their colleagues

Core Values awards
Individuals or teams nominated by colleagues

  • Together

Strategic Objectives awards
Individuals or teams nominated by colleagues 

  • Excellence in healthcare
  • Engaged, accountable colleagues
  • Trusted, open governance
  • Strong financial foundations
  • Securing the future together (nominations for external/partner organisations

 There will also be a number of additional awards presented at the ceremony including our Shining Star of the Year, the Innovation Award, the Chief Executive's Award and the Chairman's Award.

You make our Proud Awards happen so it’s really important to make your special nominations.

Colleagues
If you are a Trust colleague, and wish to nominate, please complete the Electronic Nomination Form (click here).This must be completed in Internet Explorer and will not work in Google Chrome. (Complete the fields, click ‘submit form’ in the top right corner and then complete your name and email address to send your form). 

Please check your email ‘sent’ box to ensure it has been sent. Alternatively, save a copy of your form and email it to proud@rothgen.nhs.uk.

You can also post nomination forms to the Communications Team, Junction 3, D Level, Rotherham Hospital

Patients and visitors 
If you are a patient, visitor or member of the public, and wish to nominate in the Public Recognition Award category, please click here to access the Public Recognition nomination form for patients/visitors.(Please note: patients, visitors and members of the public can only nominate in the Public Recognition category).

Our annual Proud Awards ceremony will take place during our annual Proud Week, on Thursday 8 November at MAGNA, Rotherham. 

During this week (5 – 9 November), we will also host a number of other colleague recognition and celebration events including our Long Service Awards for 40 years’ service, a Learning and Development celebration and a Volunteers celebration.
