The Rotherham NHS Foundation Trust Proud Awards 2019

Patients, carers and colleagues are being urged to nominate their health care heroes for recognition in The Rotherham NHS Foundation Trust’s 2019 Proud Awards. 

The awards celebrate excellence in healthcare by recognising Rotherham’s exceptional NHS colleagues who pull out all the stops to deliver first class community and hospital services across our borough.

To start our celebrations off,  we are pleased to officially open nominations for the Proud Awards 2019.

This year there is the opportunity to nominate within 14 categories:

Public recognition award - An individual, volunteer or team nominated by patients, visitors or members of the public

Governors award for Excellence in healthcare -
An individual or team nominated by their colleagues and the winner will be selected by our Governors

Outstanding quality in care award - An individual or team nominated by their colleagues

Team of the year (clinical) - A clinical team nominated by their colleagues

Team of the year (non-clinical) - A non-clinical team nominated by their colleagues

Our top leader - An outstanding leader nominated by their colleagues

Unsung hero -
An outstanding individual or team nominated by their colleagues

Outstanding volunteer award
- A volunteer/s nominated by staff or a fellow volunteer

Learning and Development award - A learner or colleague nominated by their teacher, mentor or a learner they have supported in their development

‘My idea made a difference’ award - A team or individual who have made improvements through a project or idea

Partnership award - Nominations for individuals or teams in external / partner organisations

Values awards
Individuals or teams nominated by colleagues

    There will also be a number of additional awards presented at the ceremony including our Shining Star of the Year, the Chief Executive's Award and the Chairman's Award.

    You make our Proud Awards happen so it’s really important to make your special nominations. 

    Patients and visitors 

    If you are a patient, visitor or member of the public, and wish to nominate in the Public Recognition Award category, please click here to access the Public Recognition nomination form for patients/visitors.If you complete the form electronically, make sure you save it before attaching it to an email. (Please note: patients, visitors and members of the public can only nominate in the Public Recognition category).

    You can submit your nomination by:

    • handing your completed form to a member of staff
    • Sending it to: FREEPOST RLXB-HECA-KEBX D Level, Foundation Trust Office, The Rotherham NHS Foundation Trust, Moorgate Road, Rotherham, S60 2UD

Colleagues

If you are a Trust colleague, and wish to nominate, please complete the electronic Nomination Form (click here).This must be completed in Internet Explorer and will not work in Google Chrome. (Complete the fields, save it and then attach the saved document to an email. Send it to rgh-tr.proud@nhs.net). 

Please check your email ‘sent’ box to ensure it has been sent.

Our annual Proud Awards ceremony will take place during our annual Proud Week, on Friday 15 November at MAGNA, Rotherham. 

During Proud Week (11-15 November), we will also host a number of other colleague recognition and celebration events including our Long Service Awards for 40 years’ service, a Learning and Development celebration and a Volunteers celebration.
