The Heart Failure Support Service role is to ensure patients are prescribed evidence-based medication at appropriate doses, to reduce mortality and improve quality of life and reduce symptoms. 

 We educate patients to help them recognise symptoms, control their condition and to use medication. Encouraging heart patients to manage their condition increases their confidence and wellbeing, preventing avoidable hospital admissions in the future.

Education and awareness 

Our Heart Failure Specialist Nurses have launched a ‘HELP’ course for all heart failure patients in Rotherham. This is a one day education and awareness course which informs patients about heart failure and the treatment programme and support available to them.
