Young Adult Transitions Team
The Rotherham NHS Foundation Trust works in partnership with Rotherham Metropolitan Council Social Services to provide support for young people with physical disabilities between the ages of 16 and 25.
Occupational Therapy
The occupational therapist can visit people in their own homes or educational setting, assess their needs and provide them with any necessary equipment or adaptations for their home.
We help the young person to work towards personal goals. These may include:
- learning to drive
- finding the right course
- working towards a certificate in education
- achieving educational qualifications
- promoting socialisation
- exploring voluntary work or employment
- moving out of the family home
- improving independence within your home
- improving independence by navigating public transport

Physiotherapy
The physiotherapist can visit people in their own homes or educational setting, assess their needs and work toward personal goals. These may include:
- improving mobility, balance, coordination and strength
- assessing and providing walking aids
- working with them to produce a personalised exercise programme, and monitor their progress
- increasing confidence to attend a local gym and advise local instructors working there on how they can best be supported
- supporting them to incorporate exercises and mobility into their college timetable
There is access to a hydrotherapy pool if this is appropriate.
