What is ‘Patient Initiated Follow Up’?

This means that you or your parent/carer can book an appointment to see us when you need one. You will be able to make an appointment in this way for 6 months after your last appointment with us. After this time, you will be discharged from the hospital care, back to your GP. You or your parent or carer can book your own follow up appointment if you need one. This might be if you are worried about your condition or symptoms, or if you feel that you are not recovering well.

Booking a follow up appointment

Telephone the Contact Centre on 01709 427949 and say you are on a ‘Patient Initiated Follow Up’ pathway.

The Contact Centre is open 8am to 6pm, Monday to Friday.

Alternatively, you can send an email to rgh-tr.contactcentrepurpleteam@nhs.net.

The appointment will be via telephone, video or face to face.

When not to use PIFU

  • If your concern is related to a different condition.
  • If it has been over 6 months since your last appointment, you will need to go back to your GP.
  • If you require urgent medical advice, you should contact your GP or NHS 111.
