What is a Personalised Care Plan?


At your booking appointment your Midwife will provide you with a Personalised Care Plan information leaflet and paper document.

The care you receive throughout your pregnancy, labour and postnatal period should be personalised to meet your needs and those of your baby and family throughout pregnancy, birth and early parenthood.

Decisions you make regarding your care because of your needs and preferences should be made in partnership with your Maternity care team to ensure you make fully informed decisions about your care.

Your Personalised Care Plans are regularly reviewed and discussed at each care contact, and can be adapted at any stage throughout your pregnancy (as your needs and preferences may change as the pregnancy progresses).

Your named Midwife and/or Obstetrician (a Doctor who specialises in pregnancy and birth) can help you to complete or adapt your Personalised Care Plans taking into account your personal preferences. Please share your preferences with them throughout your pregnancy and give consent for them to record this on your electronic patient record.

Please view the care plans as a flexible plan, as in the event that you or your baby develop more complex needs during pregnancy, labour or following birth the plan may need to be reviewed or changed, and sometimes this may look quite different from your original preferences. This is to ensure we provide you and your family with high quality, safe care at all times. However, these changes are always made in partnership with you and your family.   

 

How should I use this document?

This plan is for you and your baby. You are invited to document your wishes, concerns and preferences using the Personalised Care Plan document, which will be reviewed and discussed with you regularly throughout your pregnancy by your Midwife and/or Obstetrician.

Therefore, please keep the Personalised Care Plan document with your handheld maternity records and bring this document with you to all your appointments.

Completing the Personalised Care Plan document will support you in making informed decisions and choices around your care throughout pregnancy, labour and during the postnatal period. You are encouraged to complete your Personalised Care Plans for each pregnancy. 

Your Personalised Care Plan can be hand-written in the document provided, or you can record these on a computer and print out to place within the document if you prefer.

If you have any difficulties in reading or writing, your Midwife can support you in completing these. Please keep the Personalised Care Plan document with your handheld maternity records and bring with you to all your appointments, so you can discuss your preferences with the Maternity team.
