Maternity self referral form

If you would like to book your pregnancy with us and have a Rotherham address, you can self-refer using our online referral form and one of the Maternity Team will contact you to arrange an appointment.

You will need your NHS number to complete this referral form.

Pregnancy Advisory Service

If you would like to talk about your options for terminating a pregnancy (also known as an abortion), you can self-refer to the Pregnancy Advisory Service, based at Rotherham Hospital.

Patients, staff and local people are invited to become members so they can have a say in how our Trust is run. People from all walks of life can become members.
